Keystone Oaks Hosting COVID-19 Vaccination for Students

Keystone Oaks School District, in collaboration with Giant Eagle, will be holding a COVID-19 vaccination clinic for students age 12 and over, as well as for staff members or parents/guardians of current students who need to be vaccinated, if appointments allow. 

The first dose of the Pfizer vaccine will be administered at Keystone Oaks High School on Tuesday, June 1, 2021. The second dose will be administered on Tuesday, June 22, 2021. Both clinics will be held between 12:30 and 3:30 p.m. 

Please note that this first-dose clinic is the same date as the second-dose clinic for individuals who participated in the May 10, 2021 vaccine clinic. 

Anyone interested in receiving the vaccine must complete this pre-registration form by Tuesday, May 25, 2021. The clinic is also open to district employees who have not yet been vaccinated as well as parents/guardians of current Keystone Oaks students. Students will receive first priority for appointments. 

Next week, those who pre-registered will be sent an email with appointment information as well as a form that must be printed, completed and returned to staff at the clinic. 

Please note:

  • Students must be 12 years of age or older by June 1, 20213072 Grassmere Avenue Pittsburgh 15216 to qualify.

  • This clinic is voluntary without any recommendation, direction or coercion from anyone at Keystone Oaks School District.

  • This clinic will be facilitated by Giant Eagle and its pharmacists.

  • Keystone Oaks is not liable for any injury, loss or damage that may occur following vaccination.

  • Those scheduled for an appointment should NOT report if they or someone in their household is exhibiting any sign or symptom of COVID-19, or if they were identified as a close contact of someone diagnosed with the virus.
